In-Home Physical Therapy in Eastport

Move Better Without Leaving Your House

You shouldn’t have to choose between getting the care you need and dealing with the hassle of getting there. If mobility is already an issue, driving to appointments adds another layer of difficulty you don’t need.

That’s where in-home physical therapy changes things. Your physical therapist comes to you, evaluates your movement in your actual environment, and builds a treatment plan around your space, your routine, and your goals.

You get one-on-one attention without the waiting room. You work on balance training in the hallway you actually walk every day. You practice gait training on the stairs you need to climb. Fall prevention isn’t theoretical—it’s built around the layout of your home, the rugs you have, the furniture you navigate.

Recovery happens faster when it’s convenient. You’re more likely to stick with therapeutic exercise when it doesn’t require coordinating rides or sitting in traffic. And for people managing joint pain, neurological conditions, or post-surgery rehabilitation, that consistency makes all the difference.

Eastport Physical Therapy Services

Serving Long Island Since 2010

We’ve been providing physical therapy across Long Island for over a decade. We’re licensed, we accept Medicare and most commercial insurance, and we’ve built our reputation on showing up when we say we will.

Eastport residents deal with the same challenges as the rest of Suffolk County—aging in place, recovering from surgery, managing chronic conditions—but they also value privacy, convenience, and not having to explain their situation to a new person every visit. You get the same physical therapist each time, someone who knows your progress and adjusts your plan as you improve.

We’re affiliated with Physical Therapy Associates of Smithtown and Speonk, which means you’re working with a team that has the resources and experience to handle everything from stroke rehabilitation to pre-surgery prep. But your care still feels personal, because it is.

How Physical Therapy Works at Home

What to Expect From Your First Visit

Your first session starts with an evaluation. Your physical therapist asks about your pain, your limitations, and what you’re hoping to get back to doing. They watch how you move, test your strength and balance, and identify what’s actually causing the problem.

From there, they build a treatment plan. That might include therapeutic exercise to rebuild strength, neuromuscular re-education to retrain movement patterns, or resistance and strength training to support your joints. If you’re recovering from a fall or surgery, the focus might be on gait training and balance work to prevent it from happening again.

Each session is one-on-one. Your therapist isn’t splitting time between three other patients. They’re there to work with you, adjust exercises as needed, and make sure you’re doing them correctly. You’ll also get a home exercise program to continue between visits.

Most people see progress within a few weeks. How long you need therapy depends on your condition, but the goal is always the same: get you back to doing what you need to do, with less pain and more confidence.

Physical Therapy Services in Eastport, NY

What's Included in Your Care

You’re not just getting exercises. You’re getting a full evaluation, a personalized plan, and ongoing adjustments based on how your body responds.

Treatment might include joint pain management for knees, hips, shoulders, or backs that aren’t moving the way they used to. It could involve occupational rehabilitation if you’re trying to return to work after an injury. For people recovering from a stroke or managing neurological conditions like Parkinson’s, neurological rehabilitation focuses on retraining your brain and body to work together again.

Fall prevention is a big part of what we do in Eastport. With an aging population and homes that weren’t always designed with accessibility in mind, falls are a real risk. Balance and proprioceptive training help you stay steady on your feet. We also assess your home environment and suggest modifications that reduce risk without requiring a full renovation.

If you’re preparing for surgery or recovering from one, pre and post surgery rehabilitation gets you stronger going in and helps you heal faster coming out. Injury rehabilitation covers everything from sprains and strains to more complex trauma cases.

Medicare covers home-based physical therapy when it’s medically necessary, and most commercial plans do too. We handle the paperwork and verify your coverage before we start.

Does Medicare cover in-home physical therapy in Eastport, NY?

Yes. Medicare Part B covers home health physical therapy when your doctor orders it and you meet the homebound criteria. That doesn’t mean you’re completely bedridden—it means leaving your home requires significant effort due to illness, injury, or disability.

You’ll pay 20% of the Medicare-approved amount after you’ve met your deductible. If you have a Medicare Supplement plan, it may cover that 20%. We verify your coverage before your first visit so there are no surprises.

Most commercial insurance plans also cover home-based therapy, though your copay and visit limits will depend on your specific plan. We accept nearly all major insurers in New York and can walk you through what your plan covers during scheduling.

We treat anything that’s limiting your movement or causing pain. That includes joint pain from arthritis or overuse, balance issues that increase fall risk, and mobility problems after a stroke or neurological diagnosis.

Post-surgery rehabilitation is common—knee replacements, hip replacements, shoulder repairs, spinal surgeries. We also work with people recovering from fractures, sprains, or other injuries that make it hard to get around.

Chronic conditions like Parkinson’s, MS, or neuropathy often benefit from ongoing physical therapy to maintain strength and function. And if you’re dealing with general deconditioning—maybe you’ve been in the hospital or just haven’t been as active—we can help rebuild your endurance and confidence.

The key is that you’re having trouble leaving home for treatment. If that’s the case, we can probably help.

Most sessions run 45 minutes to an hour. That gives your physical therapist enough time to work through your exercises, make adjustments, and check your progress without wearing you out.

Your first visit is usually longer—closer to an hour or more—because it includes the full evaluation. Your therapist needs time to understand your history, test your movement, and explain the plan.

How many sessions you’ll need depends on your condition and goals. Some people see improvement in four to six weeks. Others need a few months, especially if they’re recovering from surgery or managing a chronic condition. Your therapist will give you a clearer timeline after the evaluation.

Sessions are scheduled based on your availability. Most people do two or three visits per week to start, then taper down as they get stronger.

Physical therapy focuses on movement—strength, balance, mobility, pain management. Your physical therapist helps you walk better, move more easily, and reduce pain so you can do the things you need to do.

Occupational therapy focuses on function—getting dressed, cooking, bathing, managing daily tasks. Your occupational therapist helps you adapt to limitations and find new ways to do things if your old methods aren’t working anymore.

A lot of people benefit from both. If you’ve had a stroke, your physical therapist works on getting you walking again while your occupational therapist works on getting you feeding yourself or using the bathroom independently. They’re different skill sets, but they complement each other.

We provide both services, and if your doctor orders both, we’ll coordinate your care so you’re not overwhelmed with appointments. Sometimes the same visit can include elements of both, depending on what you need.

For Medicare, yes. You need a physician’s order stating that you require home health services and meet the homebound criteria. Your doctor has to certify the plan of care, and we’ll coordinate with their office to make sure everything’s in order.

For commercial insurance, it depends on your plan. Some require a referral or prior authorization. Others allow direct access, meaning you can start therapy without seeing your doctor first. We’ll check your specific plan when you call to schedule.

Even if your insurance doesn’t require a referral, it’s still a good idea to let your doctor know you’re starting therapy. They can provide context about your condition, share relevant medical history, and adjust your treatment plan if needed. We communicate with your physician throughout your care to keep everyone on the same page.

If you’re not sure whether you need a referral, just call us. We’ll look up your plan and let you know what’s required before your first visit.

We serve Eastport and the surrounding Suffolk County communities. That includes nearby areas like Speonk, Remsenburg, East Moriches, and Manorville. If you’re within a reasonable distance from our service area, we can usually accommodate you.

Our therapists are familiar with the area and the types of homes people live in—ranch-style houses, split-levels, older homes with narrow hallways or stairs. That local knowledge helps us make practical recommendations for fall prevention and mobility improvements.

We’ve been working on Long Island since 2010, and we’ve built relationships with local physicians, hospitals, and rehab facilities. That means smoother coordination if you’re transitioning home from Stony Brook or another nearby hospital.

If you’re not sure whether we cover your specific address, just call. We’ll let you know right away if we can get to you, and if for some reason we can’t, we’ll point you toward someone who can.
