In-Home Physical Therapy Baldwin Residents Trust

You Deserve Care That Fits Your Life

You’re dealing with enough already. Pain that won’t quit. Balance issues that make you second-guess every step. Recovery from surgery that’s taking longer than anyone said it would.

Driving to appointments when you can barely get in and out of the car isn’t helping. Neither is sitting in a waiting room when you’d rather be resting at home.

That’s why we bring physical therapy to you. You get one-on-one attention in your own space, where you’re most comfortable. Your therapist sees how you actually move through your day—your stairs, your bathroom, your kitchen—and builds a plan around your real life, not a clinic floor.

Most of our patients are covered by Medicare. You’re working with licensed professionals who’ve been doing this across Nassau County for over a decade. And you’re not wasting energy on travel when that energy should go toward getting better.

Trusted Physical Therapy Services in Baldwin

We've Been Here Since 2010

We’ve been providing in-home physical therapy across Long Island since 2010, with affiliated centers like Physical Therapy Associates of Smithtown and Speonk Physical Therapy serving the community even longer.

We’re not new to Baldwin or Nassau County. We know the aging population here—over 300,000 residents age 60 and older—and we’ve built our practice around what that demographic actually needs: accessible, respectful, effective care that doesn’t require you to upend your day.

Every therapist on our team is licensed and Medicare-certified. We handle the paperwork, verify coverage, and show up when we say we will. You get continuity, not a rotating door of strangers.

How Our Physical Therapy Process Works

Here's What Happens From Start to Finish

First, we verify your insurance and schedule an initial evaluation at your home. No referral needed in New York State—you can see us directly for up to 30 days or 10 visits, whichever comes first.

During that first visit, your physical therapist assesses your mobility, strength, balance, and pain levels. They’ll ask about your goals—whether that’s walking without a cane, getting up the stairs easier, or just feeling steady on your feet again. Then they’ll map out a personalized treatment plan based on what you need, not a one-size-fits-all protocol.

Sessions typically happen two to three times a week, depending on your condition. Your therapist brings any necessary equipment and works with you using your own furniture, stairs, and layout. That’s the advantage of in-home care—you’re training in the environment where you actually need to function.

As you progress, your therapist adjusts the plan. You’ll see measurable improvements in strength, range of motion, and confidence. And when you’re ready to manage on your own, we’ll give you a home exercise program to keep up the momentum.

About Medcare Therapy Services

What's Included in Baldwin Physical Therapy

The Care You Get With Every Visit

You’re not just getting exercises. You’re getting a full evaluation of how your body moves and what’s holding you back.

That includes fall prevention and balance training—critical for Baldwin’s aging population, where falls are one of the leading causes of injury and loss of independence. We work on proprioceptive training to improve your body’s sense of position and movement, plus gait training to help you walk more safely and confidently.

If you’re recovering from surgery—knee replacement, hip replacement, rotator cuff repair—we guide you through post-surgery rehabilitation with evidence-based protocols. If you’ve had a stroke or deal with a neurological condition like Parkinson’s, we provide stroke rehabilitation and neurological rehabilitation tailored to your specific challenges.

Joint pain treatment, therapeutic exercise, resistance and strength training, neuromuscular re-education—it’s all part of the plan. And because we’re in your home, we can address real-world obstacles like narrow hallways, steep stairs, or tricky bathroom layouts that a clinic can’t replicate.

Nassau County has over 1,600 physical therapy specialists, but not all of them come to you. We do, and we’ve been doing it long enough to know what works.

Does Medicare cover in-home physical therapy in Baldwin, NY?

Yes. Medicare Part B covers physical therapy services when they’re medically necessary and provided by a licensed therapist. That includes in-home care if you have difficulty leaving your home due to mobility issues, pain, or other health conditions.

We’re a Medicare-certified provider, which means we handle the billing and verification directly. You’ll typically be responsible for 20% of the Medicare-approved amount after you’ve met your deductible. If you have a Medicare Supplement plan, that may cover the remaining balance.

We’ll confirm your coverage before your first visit so there are no surprises. And if you have commercial insurance instead of Medicare, we’ll verify that too. Most major plans cover home-based physical therapy when it’s prescribed or deemed medically necessary.

We treat a wide range of conditions, but the most common in Baldwin and Nassau County are fall-related injuries, post-surgical recovery, chronic joint pain, and neurological conditions.

If you’ve had a fall or you’re at high risk for falling, we focus on balance training, gait training, and strengthening exercises to reduce that risk. For post-surgery patients—especially after knee or hip replacements—we provide structured rehabilitation to restore range of motion, reduce pain, and get you back to normal activity.

We also work with stroke survivors and people managing conditions like Parkinson’s disease, multiple sclerosis, or neuropathy. That involves neurological rehabilitation, neuromuscular re-education, and functional training to improve coordination and independence. And if you’re dealing with arthritis, back pain, or shoulder issues, we use therapeutic exercise and joint pain treatment techniques to help you move better and hurt less.

Most sessions run between 45 minutes and an hour. That gives your therapist enough time to assess how you’re progressing, work through your exercises, and make any adjustments to your plan.

The first evaluation usually takes a bit longer—closer to an hour—because we’re gathering a full history, testing your strength and mobility, and building your treatment plan from scratch. After that, regular sessions are more streamlined but still thorough.

Frequency depends on your condition. Some patients need three visits a week early on, especially after surgery or a serious injury. Others do well with two sessions a week. As you improve, we’ll often scale back to once a week or transition you to a home exercise program you can do on your own. The goal isn’t to keep you in therapy forever—it’s to get you strong and independent again.

Not in New York State. You have direct access to physical therapy, which means you can schedule an evaluation and begin treatment without a prescription for up to 30 days or 10 visits, whichever comes first.

After that, if you need continued care, we’ll coordinate with your doctor to get a referral or treatment plan in place. But for most people, that initial window is enough time to see real progress and determine whether ongoing therapy makes sense.

If you’re using Medicare or insurance, we’ll still verify coverage and make sure everything is set up correctly on the billing side. But you don’t have to wait for a doctor’s appointment to get started. If you’re in pain, unsteady on your feet, or recovering from an injury, you can call us and we’ll get you scheduled.

You’re being treated in the space where you actually live. That means your therapist can see the real challenges you’re facing—the stairs you struggle with, the bathroom that’s hard to navigate, the chair that’s too low to get out of easily.

In a clinic, everything is set up for therapy. The equipment is ideal, the floors are flat, the lighting is bright. But that’s not your real life. At home, we can address the specific obstacles that make your day harder and teach you how to handle them safely.

There’s also the convenience factor. If you have limited mobility, chronic pain, or you’re recovering from surgery, getting to and from appointments can be exhausting or even impossible. We eliminate that barrier. You save your energy for the actual work of getting better, not for the commute.

And for many of our Baldwin patients—especially seniors—being at home just makes the whole process less stressful. You’re comfortable, you’re not exposed to other people’s germs, and you’re working one-on-one with the same therapist every time.

That depends on the type of surgery, but in many cases, you can start within a few days of getting home. For joint replacements like hip or knee surgery, early mobilization is critical. The sooner you start moving safely, the better your outcomes.

Your surgeon will give you specific guidelines, and we’ll follow those closely. Sometimes therapy starts in the hospital or a rehab facility, and we pick up once you’re discharged. Other times, we’re your first point of contact post-op.

Either way, we’ll coordinate with your surgical team to make sure the timing and intensity are appropriate. Early sessions focus on pain management, gentle range of motion, and basic strengthening. As you heal, we progress to more challenging exercises and functional movements. The goal is to get you back to your normal routine as quickly and safely as possible—without setbacks or complications.
