You stop planning your day around what hurts. Getting out of a chair doesn’t require a strategy. Walking to the mailbox stops feeling like a risk.
That’s what happens when a licensed physical therapist works with you at home, in your space, with a plan built around your body and your goals. Not a generic protocol. Not a one-size-fits-all approach.
Whether you’re recovering from surgery, managing chronic joint pain, or working to prevent another fall, the right therapy makes daily life easier. You regain confidence in your balance. You move with less hesitation. And you do it without the stress of driving to appointments or sitting in a waiting room.
Most of our patients tell us the same thing: they wish they’d started sooner. Because once you feel steadier on your feet and stronger in your routine, everything else gets a little easier too.
We’ve been providing in-home physical therapy across Long Island since 2010. We’re connected to Physical Therapy Associates of Smithtown and Speonk Physical Therapy, so you’re working with a team that’s been treating patients in this area for over two decades.
We specialize in Medicare-covered outpatient services, and we accept nearly all commercial insurances. That means fewer billing surprises and more straightforward access to the care you need.
South Hempstead has a large population of adults over 65, and many of them face the same challenge: getting to a clinic is harder than the therapy itself. That’s why we come to you. Your home is where you’ll actually be moving, recovering, and living—so it makes sense to train there.
First, we verify your insurance and schedule an initial evaluation at your home. A licensed physical therapist will assess your mobility, strength, balance, and any pain or limitations you’re dealing with. This isn’t a quick look—it’s a full evaluation to understand what’s going on and what’s realistic to improve.
From there, we build a treatment plan based on your specific needs. That might include gait training if you’re unsteady on your feet, therapeutic exercise to rebuild strength, or neuromuscular re-education if you’re recovering from a stroke or neurological condition. If fall prevention is the priority, we’ll work on balance and proprioceptive training in the actual spaces where you move every day.
Sessions typically happen one to three times per week, depending on your condition and your insurance coverage. Your therapist will also give you exercises to do between visits—because consistency is what creates results.
As you progress, we adjust the plan. The goal is always the same: help you move better, feel stronger, and do more of what matters to you without assistance.
Ready to get started?
We provide physical therapy and occupational therapy, depending on what your recovery requires. That includes fall prevention, balance training, gait training, joint pain treatment, therapeutic exercise, resistance and strength training, stroke rehabilitation, neurological rehabilitation, and pre- and post-surgery rehabilitation.
If you’ve had a hip replacement, knee surgery, or you’re managing arthritis, we’ll focus on restoring range of motion and building strength safely. If you’ve had a stroke or deal with a neurological condition like Parkinson’s, we’ll work on coordination, movement patterns, and reducing your risk of future falls.
In South Hempstead and across Nassau County, falls are one of the leading causes of injury for older adults. That’s why so much of what we do centers on fall prevention. We assess your home environment, work on your balance in real-world scenarios, and teach you how to move more confidently.
Most of our services are covered by Medicare, and we handle the paperwork. You’ll know what’s covered before we start, and we’ll work within those guidelines to get you the most benefit possible.
Yes. Medicare Part B covers physical therapy when it’s medically necessary and prescribed by your doctor. That includes in-home therapy if you’re homebound or if leaving your home requires a considerable and taxing effort.
We’re a Medicare-certified provider, so we handle the verification and billing directly. Before your first session, we’ll confirm your coverage and let you know if there’s any out-of-pocket cost based on your plan.
Most patients with Medicare pay little to nothing after their deductible is met. If you have a Medicare Advantage plan, coverage works similarly, but we’ll verify your specific benefits ahead of time so there are no surprises.
If you’re avoiding activities because of pain, if you’ve fallen or feel unsteady, or if you’re recovering from surgery or an injury and not progressing as expected—those are all signs that physical therapy will help.
A lot of people wait too long because they assume things will improve on their own. Sometimes they do. But often, waiting just means you’re compensating with poor movement patterns, which leads to more pain or another injury down the line.
Physical therapy gives you a structured plan to rebuild strength, improve balance, and move correctly. It’s not about pushing through pain—it’s about retraining your body so you can do normal activities without fear or discomfort. If you’re unsure, we can do an evaluation and give you an honest assessment of whether therapy makes sense for your situation.
Physical therapy focuses on movement: strength, balance, mobility, and pain reduction. If you’re recovering from surgery, dealing with joint pain, or working to prevent falls, that’s typically physical therapy.
Occupational therapy focuses on daily activities: getting dressed, cooking, bathing, and other tasks that require fine motor skills and coordination. If you’ve had a stroke or you’re struggling with tasks that involve your hands or cognitive function, occupational therapy is usually the better fit.
We offer both, and sometimes patients benefit from a combination. Your doctor or our therapist will recommend what makes the most sense based on your goals. The good news is that both are covered by Medicare when medically necessary, and both can be done in your home.
Most patients start noticing improvement within two to four weeks, but that depends on your condition, your consistency, and how your body responds to treatment.
If you’re recovering from surgery, you might need several weeks or a few months of therapy to regain full function. If you’re working on fall prevention or balance training, you might see progress faster—especially if you’re doing your home exercises between sessions.
The national success rate for physical therapy is around 68 to 72 percent, and 79 percent of patients report significant pain reduction after treatment. Those numbers improve when patients stick with the plan and stay engaged in their recovery. We’ll give you a realistic timeline during your evaluation, and we’ll adjust as you progress.
Your first visit is an evaluation. Your therapist will ask about your medical history, current symptoms, and what you’re hoping to improve. Then they’ll assess your strength, balance, range of motion, and how you move during everyday activities.
This usually takes about an hour. We’ll also look at your home environment to identify any fall risks or obstacles that might be affecting your mobility.
At the end of the session, your therapist will explain what they found and outline a treatment plan. You’ll know how many sessions per week we’re recommending, what types of exercises or interventions we’ll use, and what kind of progress you can expect. If you have questions about coverage or scheduling, we’ll answer those too.
Yes. In-home therapy is different because you’re working in the environment where you actually live. That means we can address real-world challenges—like navigating your stairs, getting in and out of your shower, or moving safely around furniture.
A lot of patients do better at home because there’s no commute, no waiting room, and no pressure to perform in an unfamiliar setting. You’re more relaxed, and we can tailor everything to your specific space and routine.
If you tried clinic-based therapy and didn’t get the results you wanted, it’s worth trying again at home. The approach is more personalized, and for many people, that makes all the difference.
