In-Home Physical Therapy in Greenport

Recover Faster in Your Own Space

Getting to a clinic twice a week sounds simple until you’re the one who can’t drive after surgery. Or you’re managing balance issues that make every car ride feel risky. Or you’re caring for someone whose energy is spent by noon.

That’s where in-home physical therapy changes everything. You get the same licensed, skilled care you’d receive in a clinic, but your therapist comes to you. No arranging rides. No rushing to make appointment times. No sitting in waiting rooms when you’d rather be resting.

Your treatment happens in the environment that matters most—the one where you actually live. Your therapist sees the stairs you need to climb, the bathroom you need to navigate, the kitchen where you need to move safely. That means your exercises and strategies are built for your real life, not a generic clinic setup.

You recover in the place where you’re most comfortable. And that comfort matters more than most people realize when you’re trying to heal.

Trusted Physical Therapist Serving Greenport

Serving Long Island Since 2010

We’ve been providing in-home physical therapy across Suffolk and Nassau County since 2010. We built our practice around one specific need: helping people who struggle to leave home get the therapy they actually need.

Our therapists are licensed professionals who accept Medicare and most commercial insurance. We work with patients recovering from surgery, managing chronic conditions, rebuilding strength after a stroke, or dealing with balance and fall risk—which matters especially here on Long Island, where Suffolk County ranks 5th in New York State for fall-related incidents among older adults.

We’re connected to Physical Therapy Associates locations in Smithtown and Speonk, which means you’re working with a team that has deep roots in this community. You’re not getting a rotating cast of strangers. You’re getting consistent, personalized care from people who know the North Fork and understand what local families are dealing with.

How In-Home Therapy Works in Greenport

What to Expect From Your First Visit

Your first session starts with a full evaluation. Your physical therapist will assess your mobility, strength, balance, and any pain or limitations you’re experiencing. They’ll also look at your home environment—stairs, furniture layout, bathroom setup—to understand what you’re working with day to day.

From there, your therapist designs a treatment plan based on what you need to accomplish. Maybe that’s regaining strength after a hip replacement. Maybe it’s improving balance so you feel safe walking to the mailbox. Maybe it’s reducing joint pain so you can cook dinner without discomfort. Your plan is yours.

Each session after that builds on the last. You’ll do therapeutic exercises, work on gait training if needed, and learn techniques you can practice between visits. Your therapist will adjust your program as you improve, and they’ll prescribe home exercises that fit your schedule and abilities.

Most patients see us one to three times per week depending on their needs and insurance coverage. Sessions typically last 45 minutes to an hour. And because we come to you, there’s no travel time eating into your day or energy.

About Medcare Therapy Services

Physical Therapy Services in Greenport, NY

What's Included in Your In-Home Care

Our physical therapists provide a full range of services depending on what you’re recovering from or managing. That includes fall prevention and balance training—critical on Long Island, where 88% of injury hospitalizations for adults over 65 in Nassau County are due to falls. We also offer gait training to help you walk more safely and confidently.

If you’re recovering from surgery, we provide pre- and post-surgery rehabilitation to restore your strength and mobility. For stroke patients, we offer neurological rehabilitation that focuses on rebuilding movement and function. We treat joint pain, provide therapeutic exercise and resistance training, and work with people managing chronic conditions like diabetes or cardiac issues.

We also coordinate occupational therapy when needed, so if you’re struggling with daily tasks like dressing, bathing, or cooking, we can bring in additional support. Everything is designed to help you function better in your own home, not in a clinic that doesn’t look anything like your real life.

And because we accept Medicare and nearly all commercial insurances, most of our patients pay little to nothing out of pocket. We handle the billing and verification so you can focus on getting better.

Does Medicare cover in-home physical therapy in Greenport, NY?

Yes. Medicare Part B covers outpatient physical therapy when it’s provided by a licensed therapist and deemed medically necessary by your doctor. That includes in-home therapy if you’re homebound or have difficulty leaving your house due to illness, injury, or mobility issues.

You’ll typically be responsible for 20% of the Medicare-approved amount after you’ve met your deductible. But many people also have supplemental insurance that covers that remaining portion, which means you may owe nothing.

We accept Medicare and handle all the billing and authorization directly. Before we start treatment, we’ll verify your coverage and let you know what, if anything, you’ll be responsible for. No surprises.

We treat a wide range of conditions, but the most common include post-surgical rehabilitation (hip replacement, knee replacement, spinal surgery), stroke recovery, balance and fall prevention, joint pain from arthritis or overuse, and general weakness or deconditioning after illness or hospitalization.

We also work with patients managing neurological conditions like Parkinson’s or multiple sclerosis, people recovering from fractures or injuries, and anyone who needs gait training to walk more safely. If you’ve been told you need physical therapy but can’t easily get to a clinic, there’s a good chance we can help.

Your doctor will typically provide a referral or prescription for therapy, which gives us the clinical direction we need to build your treatment plan. From there, we customize everything based on your specific goals and limitations.

The biggest difference is convenience and context. You don’t have to arrange transportation, get dressed and out the door, or spend energy traveling when you’re already tired or in pain. Your therapist comes to you, which removes a massive barrier for a lot of people.

But there’s also a clinical advantage. Your therapist sees your actual living environment—the stairs you climb, the bathroom you use, the furniture you navigate around. That means the exercises and strategies we teach are tailored to your real life, not a generic gym setup. If you’re working on balance, we’re training you on the surfaces and spaces you’ll actually encounter every day.

You also get one-on-one attention for the full session. There’s no splitting time with other patients or waiting for equipment. It’s just you and your therapist, focused entirely on your progress.

It depends on what you’re recovering from and how quickly you progress. Some patients only need a few weeks of therapy after a minor surgery or injury. Others may need several months, especially if they’re recovering from a stroke or managing a chronic condition.

Most people start with two to three sessions per week. As you get stronger and more independent, we typically reduce frequency—maybe down to once a week, then every other week, and eventually discharge you once you’ve met your goals.

Medicare and insurance companies do have limits on how much therapy they’ll cover in a given period, but we monitor that closely and work within those guidelines. If you need ongoing maintenance or support beyond what insurance covers, we’ll talk through your options before you’re ever surprised by a bill.

Not much. Wear comfortable clothing that allows you to move—something you’d wear to exercise in. If you have any recent imaging (X-rays, MRIs), surgical notes, or a list of your current medications, it’s helpful to have those available, but not required.

Make sure there’s a clear space where your therapist can work with you. That might be a living room, bedroom, or any area with enough room to move safely. If you use a walker, cane, or any other assistive device, have that nearby.

Your therapist will bring any equipment they need for the session. You don’t need to buy anything in advance. After your evaluation, if there’s something that would help your progress—like resistance bands or a specific type of support—your therapist will let you know and we’ll either provide it or tell you where to get it.

Yes, and it’s one of the most important services we offer. Falls are the leading cause of injury-related death for adults over 65, and here on Long Island, the rates are higher than most of the state. Suffolk County ranks 5th in New York for fall prevalence, which makes fall prevention a critical focus for anyone experiencing balance issues or weakness.

Our fall prevention program includes balance training, proprioceptive exercises (which help your body understand where it is in space), gait training, and strength work targeting the muscles that keep you stable. We also assess your home for fall risks—loose rugs, poor lighting, tricky stairs—and recommend modifications that make your space safer.

The goal is simple: keep you on your feet and confident moving around your own home. Most people see noticeable improvement in their balance and stability within a few weeks, and that translates directly into fewer close calls and a lot more peace of mind.
