Getting to a clinic when you’re already dealing with pain, balance issues, or recovery from surgery isn’t just inconvenient. It’s often the reason people skip appointments or stop treatment altogether.
In-home physical therapy removes that barrier. You get the same professional care, the same equipment, and the same results—but in your own space, on your schedule, without the stress of transportation or waiting rooms.
That means better attendance, more consistent progress, and real improvements in how you move through your day. Whether you’re recovering from a fall, managing joint pain, or rebuilding strength after surgery, treatment happens where you’re most comfortable. And when you’re comfortable, you’re more likely to stick with it.
You don’t lose independence because you can’t get to a clinic. You keep it because care comes to you.
We’ve been providing in-home physical therapy and occupational therapy across Long Island for over a decade. We work with Medicare beneficiaries and accept nearly all commercial insurance plans.
Our therapists are licensed, experienced, and equipped with the same tools you’d find in a clinic—portable bikes, resistance bands, balance boards, ultrasound units, and more. We bring everything needed to assess your condition, create a personalized plan, and help you make real progress.
Center Moriches has a large senior population, and many of our clients here are managing chronic pain, recovering from surgery, or working to prevent falls. We understand what it’s like to need care but struggle with mobility or transportation. That’s why we built our service around coming to you.
Your first session starts with a thorough assessment. We look at your mobility, strength, balance, range of motion, and pain levels. We also talk through your goals—what you want to be able to do again, what’s been getting harder, and what’s holding you back.
From there, we build a treatment plan specific to your needs. That might include gait training if you’re unsteady on your feet, therapeutic exercise to rebuild strength, neuromuscular re-education after a stroke, or joint pain treatment to improve how you move. Every plan is different because every person is different.
Sessions typically happen one to three times per week depending on your condition and insurance coverage. Each visit is hands-on. We guide you through exercises, adjust your plan as you improve, and make sure you understand what you’re doing and why.
You don’t need a prescription to start in New York State—you can be seen by a physical therapist for up to 30 days or 10 visits without one. After that, we coordinate with your doctor if ongoing care is needed. Most of our clients are covered through Medicare or their insurance plan, and we handle the paperwork.

We treat a wide range of conditions, and everything we do is designed around helping you function better at home. That includes fall prevention and balance training, which are critical for older adults in Center Moriches where uneven sidewalks, stairs, and seasonal weather can increase fall risk.
If you’re dealing with joint pain—whether it’s your knees, hips, shoulders, or back—we use a combination of manual therapy, strengthening exercises, and pain management techniques to reduce discomfort and improve mobility. For stroke survivors or those with neurological conditions, we provide stroke rehabilitation and neuromuscular re-education to help you regain coordination and movement control.
Pre and post surgery rehabilitation is another major focus. Whether you’ve had a knee replacement, rotator cuff repair, or spinal surgery, we help you recover faster and avoid complications by guiding you through the right exercises at the right pace. We also work with people managing chronic conditions like arthritis, Parkinson’s, or COPD who need ongoing support to maintain their independence.
Each session is built around your goals. Some people want to walk without a cane again. Others want to get up and down stairs safely, or be able to garden, or just move without pain. Whatever matters to you, that’s what we work toward.
Yes. Medicare Part B covers outpatient physical therapy when it’s medically necessary, and that includes in-home treatment. You don’t need to be homebound to qualify—you just need a condition that requires skilled therapy.
Most of our clients in Center Moriches use Medicare, and we handle the billing and documentation directly. There’s typically a copay or coinsurance depending on your plan, but the majority of the cost is covered. If you have a Medicare Advantage plan or supplemental insurance, coverage may be even broader.
We also accept most commercial insurance plans. Before your first visit, we verify your benefits so you know exactly what to expect. No surprises, no runaround.
We treat anything that affects your ability to move, function, or stay safe at home. That includes recovery from surgery, injury rehabilitation, chronic joint pain, arthritis, balance problems, and neurological conditions like stroke or Parkinson’s.
A lot of our clients in Center Moriches come to us after a fall or because they’re afraid of falling. We do a full balance assessment and create a fall prevention plan that includes strength training, gait training, and proprioceptive exercises to improve stability. We also work with people recovering from hip or knee replacements, rotator cuff repairs, and spinal surgeries.
If you’ve had a stroke or deal with nerve damage, we provide neuromuscular re-education to help retrain your muscles and improve coordination. And if you’re managing a chronic condition that’s making daily tasks harder, we build a maintenance plan to keep you moving and independent.
Most sessions last between 45 minutes and an hour. That gives us enough time to work through your exercises, address any new concerns, and make adjustments to your plan as you progress.
The frequency depends on your condition and your insurance. Some people start with three sessions per week and taper down as they improve. Others need ongoing weekly visits to manage a chronic condition or maintain strength and balance.
We don’t rush through appointments. You get the full time, and you’re not competing with other patients or waiting for equipment. It’s just you and your therapist, focused on your goals.
Not in New York State. You can see a physical therapist without a prescription for up to 30 days or 10 visits, whichever comes first. After that, if you need continued care, we coordinate with your doctor to get a referral.
This is called direct access, and it’s designed to get you help faster. If you’re dealing with pain, balance issues, or mobility problems, you don’t have to wait for a doctor’s appointment before starting treatment. You can call us, schedule an evaluation, and begin therapy right away.
That said, we do communicate with your physician throughout your care. We send progress notes, update them on your treatment plan, and make sure everyone’s on the same page. If your doctor has already given you a referral, even better—we’ll take it from there.
We bring professional-grade equipment to every session. That includes resistance bands, weights, balance boards, vestibular discs, portable therapy bikes, and tools for manual therapy. If your treatment plan calls for electrical stimulation or ultrasound therapy, we bring those units too.
You don’t need to buy anything or set up a home gym. We provide everything required for your specific treatment plan. Some exercises use items you already have at home—a sturdy chair, a countertop for balance support, or a few steps for gait training. But all the specialized equipment comes with us.
Our goal is to replicate what you’d get in a clinic, just without the commute. You get the same quality of care, the same hands-on guidance, and the same results—all in the comfort of your own home.
Most people start noticing improvements within two to four weeks, but it depends on your condition, your consistency, and how your body responds to treatment. Some changes happen quickly—less pain, better balance, more confidence moving around. Others take longer, like rebuilding strength after surgery or retraining movement patterns after a stroke.
Physical therapy isn’t a quick fix. It’s a process. But it works. Studies show success rates between 68% and 72% for reducing pain and improving function, and most patients report positive outcomes when they stick with their plan.
We track your progress at every session. If something isn’t working, we adjust. If you’re improving faster than expected, we advance your exercises. The key is showing up and doing the work—and when therapy happens at home, that’s a lot easier to do.
